We say the Sacrament of the Altar because it takes place on the altar. At the Last Supper, the Lord Jesus changed the bread and wine into His body and blood. Then he said to his apostles. Could you please do this in remembrance of me? With these words, He gave the apostles the power to change the bread and wine.
